What I’m Working On

I did a class for a little company in Toledo on Problem Solving, 5 Why.

What I learned: Why is a powerful tool for root cause analysis. Why is a hard-ass question. It’s accusatory to most Americans. How can I do better / not lose 5 Why power while acculturating to the purpose?

Root Cause means Take it to the end. Branch. Cross functions and levels of the product / process and organization. You many not finish, you may create new projects that are driven by the original occurrence but support having to NEVER fix the same occurrence again.

The end of Root Cause Analysis has to be PREVENTION. We’ve gone DEEP and WIDE.

After we root cause the issue, we don’t solve the occurrence, we have prevented the problem through irreversible corrective action throughout the company and processes and procedures and product lines.

We DON’T LEAVE MONEY ON THE TABLE!
